I'm not quite sure I understood your requirements fully.
The size of the page can be specified explicitly at
fo:simple-page-master element using page-width/page-height properties.
If you know your advertisement size beforehand you can generate
appropriate XSL-FO using XSLT stylesheet which, for example, takes
advertisement size as a parameter.
