I have fixed the index issue by adding the parameter <xsl:param name="xep.extensions">1</xsl:param>.
Hope that's the right way to do it.
I am still figuring out a way to take care of the <programlisting> tag within the <table> tag.

We have been experiencing a couple of issues with this FO file and it will be great if you could have a look at it.
Please look at the PDF generated from the attached FO using XEP.
Issue #1:

If you look at the index, you can see that some terms on the left coplumn overlap into the right column.
E.g.
DBMSs (database management systems) on page 547
Mysql_connect() function (PHP) on page 550
Any idea as to why this happening? Looks like the page breaks are not being applied correctly.
Issue #2:
---------------
Table on page# 103. The left hand column overlaps into the right hand column.
After further investigation I found that the the left column text is a <programlisting> tag and that's why it is centered to the page.
How can we disable that when it is enclosed in a table?
Your help is greatly appreciated and I look forward to your reply soon.
