Generally speaking, you can use fo:conditional-page-master-reference
for that sort of thing: you can specify one page-master for the first
page, one for the last page, and one for pages inbetween, for example.
Your specific case is made more difficult, though, by the fact that the
format of the first page depends on the length of the document (footer
only for single-page documents). I don't know how you would handle
that automatically.
