It's not very pretty but you can solve this by using multiple block
containers such that you end the first block container before the span,
start a new one that spans, then put the data after the span in a third
block container.
XSL-F0 1.0 is ambiguous about what it means to specify writing mode on
fo:simple-page-master, in particular, whether or not it establishes the
writing mode of the flow's content.
in FO 1.1 this ambiguity has been corrected so that you'll be able to
unambiguously set the writing mode at the page master level, eliminating
the need to use block containers in this way.
