The DocBook xref element is rendered as an fo:basic-link element in XSL-FO,
which is supported in XEP. This is a stylesheet issue, best asked on the
docbook-apps mailing list. Normally xrefs work fine, so there may be
something about a stylesheet customization or usage in the document.
