Steve, if you can accept the "continued" text to be in a page header
or footer, this can be done with standard XSL-FO using
markers. Define the marker to be "continued" at the start of the
table, then define it to be "" at the end of the table, and at the
bottom of the page (say) you can retrieve the last one from the page
and get the effect you need.
But in XSL-FO 1.0 this cannot be done in the table itself. It can be
done in the XSL-FO 1.1 in the table.
