[xep-support] Lost page-number-citation

Dear support,
 
In very few cases the page-number-citation gets lost in the middle of documents. The ref-id's are well placed in the FO/PDF. Rendering the same FO again mostly comes out well. Are there any known circumstances except missing ref-id's which makes this occur? Numbers are replaced by '?'.
