I think Terry is correct here. I can take a clue from the XSL that tries to convert old version 3 files to the version 4 format. The test document has the structure below. This leads me to the conclusion that you only have one font entry for every font. You would need to create a unique name for the font and use that exact font name.

version 3 had a fonts.xml file, entries looked like.

You may have to create a separate <font> entry for each style, and reference them from the FO as such, eg.
font-family="TheSerifB-W4SemiLightItalic".
